I entrust them in the care of the staff and volunteers of First United Methodist Church of Miami. I understand there may be scenarios and/or circumstances when staff and/or volunteers may need to transport my child. In the case that transport is needed and that these staff volunteers have been fully background checked, trained in United Methodist Children and Youth Protection Policy and are covered under our church and/or conference insurance, I grant permission for First United Methodist Church of Miami to transport them to the appropriate destination(s) which have been communicated and acknowledged here below.
